What is chronic disease management?
A chronic disease is a health condition that lasts a year or longer and needs ongoing medical care, such as type 2 diabetes or high blood pressure. Chronic disease management is the regular care that keeps the condition under control: checkups, lab tests, medicines, lifestyle changes and early treatment of problems.
At Texas Primary Care Associates, the same providers follow you from visit to visit. They track your numbers over time, adjust your treatment and coordinate with specialists when you need one, so your care stays connected.
- Personal goals for blood sugar, blood pressure and cholesterol
- Lab work ordered and reviewed at your visits
- Medication reviews, dose changes and refills
- Referrals to specialists when your condition needs one

Blood pressure, weight and lab tests such as A1C, cholesterol or thyroid levels show how well treatment is working.

Together you set targets and make changes to medicines, diet or activity when needed.
Follow up on schedule
You return on a set schedule, with refills and referrals handled along the way.
How are common chronic conditions checked?
Each condition has its own key measurements. Your provider decides how often to check them based on your health and treatment.
| Key test or measurement | What it shows | What you can do at home | |
|---|---|---|---|
| Type 2 diabetes | A1C blood test, usually at least twice a year | Your average blood sugar over about the past three months | Check blood sugar if advised, take medicines as prescribed, stay active |
| High blood pressure | Blood pressure reading at each visit | Whether your blood pressure is at your goal | Take home readings and bring them to your visit |
| High cholesterol | Lipid panel (cholesterol blood test) | LDL, HDL and triglyceride levels and your heart risk | Eat more fiber and less saturated fat, take medicine if prescribed |
| Thyroid disease | TSH blood test, rechecked after dose changes | Whether your thyroid dose is right | Take thyroid medicine the same way every day |
| Asthma | Symptom review and inhaler check | How well your asthma is controlled | Follow your asthma action plan and avoid triggers |
Living well with a long-term condition
Chronic conditions often cause no symptoms until they lead to a problem, such as a heart attack, stroke, kidney damage or vision loss. Regular visits catch changes early, while they are easier to treat. Many conditions are linked, so high blood pressure, high cholesterol and diabetes are often managed together.
What happens at a chronic care visit
- A review of symptoms, home readings and any new concerns
- Vital signs, including blood pressure and weight
- Lab work when it is due, or a review of recent results
- Medication review, refills and dose changes
- Updates to your goals and your next follow-up date
How to get the most from each visit
- Bring all of your medicine bottles or an up-to-date list
- Bring your blood pressure or blood sugar log if you keep one
- Write down symptoms, side effects or questions before you come
- Ask whether you need to fast before lab work

When a specialist is needed
Most chronic conditions can be managed in primary care. If your condition is complex or not responding to treatment, your provider can refer you to a specialist, such as a cardiologist, endocrinologist or kidney specialist, and keep coordinating your overall care.
Insurance and cost
Texas Primary Care Associates accepts most major insurance plans, including Blue Cross Blue Shield, UnitedHealthcare, Medicare and Ambetter. Chronic care visits and lab work usually involve your plan’s copay or deductible. See insurance accepted or call to verify coverage.
